Discharge Summary SystemDischarge summaries are one of the most frequently completed forms within a health service. They need to be completed whenever a patient is discharged or transferred from a hospital or health service. But their usefulness deteriorates significantly if a discharge form isn't completed and distributed at the time of discharge, which is frequently the case. By converting the discharge form into an electronic system, the information can much more easily be distributed to GPs and other medical practitioners at the time of discharge and the information therefore becomes much more useful. Call The IT Hive to find out how we can create a custom discharge summary system for your organisation. |

Incident ReportingOne of the keys to effective incident management is receiving information about incidents in a timely way. Our solution for incident reporting was to provide access to the electronic submission form to the staff who would be reporting the incident. So rather than having staff complete paper-based forms and leaving it to reception to complete, to enter the data onto the incident management system, we put the electronic forms directly in front of the relevant staff. This means that the data from incident reports is available immediately to management and occupational health representatives. Our solution uses the best features of web-based forms to keep the processes simple and fast as possible. |

Incident ManagementThe other aspect to effective incident management is a good system for searching and sorting incident reports as well as enabling tracking of incident follow-ups and action taken. Our system provides a full audit trail of all recommendations and actions that have been taken. Although our system allows for exporting of incident data we've built the most useful reports and statistics right into it so you can access statistics to support ACHS benchmarks very quickly and easily. One of the advantages of a custom-developed incident management system is that you can have the system fine-tuned to suit the unique needs of your organisation exactly. Call us to find out more. |

Patient Allocation SystemWe created this system to help facilitate the everyday patient management tasks on a busy ward. It allows any staff to quickly see a list of patients on the ward at any time, for coordinators to allocate patients to nurses, and for these allocations to be saved and retrieved at future dates. Commonly used nursing team documents such as handover sheets, sleep charts, and coordinator's reports are produced using the latest data just by clicking a button. Clicking on any of the patient names reveals a full set of options and reports associated with that patient providing quick and easy access to all of the available information for each patient. The system also allows for coordinators to view tasks that need completing for any nurse's group of allocated patients. |

Self-Completed Patient Outcomes Data Collection SystemThis system was created for a psychiatric hospital that needs to collect a lot of questionnaire data from patients at the time of admission and discharge. Previously these questionnaires were completed on paper and then transferred by a reception staff onto an Excel spreadsheet. The new system allows patients to answer the questions directly on the computer themselves and this makes the result of their questionnaires available immediately for analysis and reporting. The results of the questionnaires can be used by clinical staff to help formulate care plans. The administrative section of this system allows for searching and reporting of the stored data in a large number of ways. For example, reports required by the health department on a quarterly basis can be produced easily just by selecting the period and clicking a button. In the same way, patient satisfaction data can be viewed at any time including average satisfaction values for any of the major areas examined within the hospital. |

Patient Risk Assessment AdministrationThe main benefit of submitting the patient risk assessment data onto an electronic system is the fact that it allows immediate access to stored information both at the time of admission and any time during the patient's stay. The data collected can also be used to contribute to the care planning process and can be used to generate reports that summarise and average the results of population at any time. This can be very useful for such things as determining staffing needs and ensuring the ward is neither over- nor understaffed. |

Patient Risk AssessmentFor this system, we took the paper-based system which was working well and just converted it to an electronic system. There's no particular time savings for the nurses completing this form (although it doesn't take any longer than the previous form), but the major benefit is that the assessment is scored immediately. The results are formatted into a legible, accurate document for printing and the data is available immediately for analysis or review by both clinical and management staff. This simple user interface provides access to violence, suicide, and falls risk assessment and takes less than 5 minutes to complete yet contributes greatly to patient safety and care. |

Staff Rostering SystemThe rostering system in some ways acts like a spreadsheet in that it allows for selection of values for each staff member for each shift and automatically calculates the number of hours that staff member has worked per fortnight and the number of staff rostered on for each day. |

Wordpress for TherapistsWe have created a series of template based websites that use Worpress as a simple CMS platform. This design and the following three are part of our 'Wordpress for Therapists' collection, that we have created in order to provide a high quality, low cost entry option for therapists wanting to establish a web presense. Wordpress is an excellent platform for this type of site becase it allows for the therapist to manage their own content - adding & editing text or images as they wish. It has been used as a blogging platform for many years but has the power and simplicity to operate as a more comprehensive content management system. We see this type of CMS based website becoming much more widely used over coming years. |
